How Does Medicare Advantage Work?
Medicare Advantage (also called Medicare Part C) is a private plan that takes the place of Original Medicare. You still have Medicare. You just get your benefits through an insurance company instead of straight from the government.
Here’s what usually comes in one plan:
Hospital stays, skilled nursing, hospice
Doctor visits, outpatient care, lab work, preventive care
Prescription drugs (most plans include it)
Many plans add dental, vision, hearing, or fitness benefits that Original Medicare doesn’t cover
So what’s the catch? You’ll usually need to use the plan’s network of doctors and hospitals. And you still pay your Part B premium, which is $202.90 a month in 2026.
Who Sells Medicare Advantage Plans in Oregon?
For 2027, nine insurers sell regular Medicare Advantage plans in Oregon. Which ones you can buy depends on your county, because plans are approved county by county.
In the Portland area, Multnomah, Washington, and Clackamas counties each have 32 plans from 8 insurers to choose from. (Yes, 32. No wonder people call for help.)
| Insurer | 2027 plan types in Oregon | Oregon counties served | Plans in Multnomah County |
|---|---|---|---|
| Devoted Health | HMO, PPO | 19 | 5 |
| Humana | HMO, PPO | 13 | 6 |
| ATRIO Health Plans | PPO | 11 | 2 |
| UnitedHealthcare | HMO-POS, PPO | 11 | 5 |
| Wellcare | HMO-POS, PPO | 11 | 5 |
| Kaiser Permanente | HMO-POS | 10 | 3 |
| Regence BlueCross BlueShield of Oregon | HMO, PPO | 8 | 4 |
| PacificSource Medicare | HMO-POS | 8 | 0 |
| Aetna Medicare | HMO | 7 | 2 |

A few other Oregon insurers, including CareOregon Advantage, AllCare Advantage, Samaritan Advantage, AgeRight Advantage, and SCAN, offer only special needs plans for 2027. Those are for people who qualify for both Medicare and the Oregon Health Plan, live in a care facility, or have certain chronic conditions.

What Does Medicare Advantage Cost in Oregon?
A $0 premium sounds free. It isn’t quite. Here’s what you actually pay with a Medicare Advantage plan:
- 1
Your Part B premium.
You keep paying this no matter what. It’s $202.90 a month in 2026.
- 2
The plan premium.
Many Oregon plans charge $0. Others charge a monthly amount for richer benefits or a wider network.
- 3
Copays and coinsurance.
You pay a set amount for doctor visits, specialists, imaging, and hospital stays.
- 4
Drug costs.
Your share depends on the plan’s drug list. In 2027, the Part D deductible can be up to $700.
The caps that protect you
| Cap (2027) | Limit | What it covers |
|---|---|---|
| In-network out-of-pocket maximum | $9,850 | Copays and coinsurance for Part A and Part B care from network providers |
| PPO combined maximum | $14,800 | In-network and out-of-network care together |
| Part D out-of-pocket cap | $2,400 | Covered prescription drugs |
Those are the ceilings set by Medicare. Plenty of plans set their caps well below them, which is one of the biggest differences between plans. Drug costs are their own puzzle, so here’s how Part D drug plans work in Oregon.
What it looks like in the Portland area for 2027: in Multnomah, Washington, and Clackamas counties, 12 of the 32 plans charge a $0 premium. Premiums run from $0 to $244 a month, and in-network out-of-pocket maximums run from $4,000 to $9,250.

HMO vs PPO: Which Network Fits You?
Most Medicare Advantage plans in Oregon are HMOs or PPOs. The difference comes down to one question: how much freedom do you want in picking doctors?
| HMO | PPO | |
|---|---|---|
| Doctors | Must use the plan’s network | Can go out of network, for more money |
| Referrals | Usually needed to see a specialist | Usually not needed |
| Cost | Often lower premiums and copays | Often higher premiums |
| Out-of-pocket cap | In-network only | In-network plus a higher combined cap |
| Best for | People happy with one local health system | People who travel, or who see specialists across systems |
In the Portland area, this choice often comes down to your health system. If your doctors are at Providence, Legacy, OHSU, or Kaiser, check which 2027 plans include them before anything else. Networks change every year, so last year’s answer may not be this year’s. Switching plans is easy. Switching doctors you love is not.

Pros and Cons of Medicare Advantage
No plan is perfect for everyone. Here’s the honest scorecard.
What people like
- ✓Low or $0 monthly premiums on many plans
- ✓A yearly cap on medical costs, which Original Medicare alone doesn’t have
- ✓Drug coverage built in, so one card does it all
- ✓Extras like dental, vision, hearing aids, and gym memberships
What trips people up
- !Networks limit which doctors and hospitals you can use
- !Prior authorization: the plan may need to approve some care first
- !Costs add up through copays if you need a lot of care
- !Coverage away from home can be thin, especially on HMOs
- !Plans change every year, so last year’s great plan may not be this year’s

When Can You Enroll or Switch?
You can’t join a Medicare Advantage plan any old day. There are set windows, and missing one can mean waiting a year. Turning 65 soon? Start with our Turning 65 in Oregon checklist. Retiring earlier? Here’s how to bridge your health insurance until Medicare.

|---|---|---|
| Initial Enrollment Period | 7 months: the 3 months before the month you turn 65, your birthday month, and the 3 months after | Join a Medicare Advantage plan for the first time |

How to Choose the Best Medicare Advantage Plan in Oregon
What’s the “best” plan? Honestly, the one that fits your doctors, your prescriptions, and your budget. Here’s the order to check things in:
- 1
Your doctors and hospital. Are they in the network? Start here, because everything else is easier to fix.
- 2
Your prescriptions. Is each drug on the plan’s list, and at what tier? A drug in the wrong tier can cost hundreds a year.
- 3
The out-of-pocket max. Look at your worst-case year, not just your best one.
- 4
The premium. Now compare monthly costs between the plans still standing.
- 5
Star ratings. Medicare rates plans from 1 to 5 stars on quality and service.
- 6
The extras. Dental, vision, and hearing are nice tie-breakers. They shouldn’t be the main reason you pick a plan.
