PacificSource Medicare: 2027 Medicare Advantage Plans in Oregon
Written by Kelly Raulerson · Last updated FacebookEmail
Key Takeaways
- ✓The lineup shrank. PacificSource sold ten regular Medicare Advantage plans in Oregon in 2026. For 2027 it sells four.
- ✓Portland is out. PacificSource has no Medicare Advantage plan in Multnomah, Washington, or Clackamas counties for 2027.
- ✓Your 2026 plan may have a new name, or no replacement. Check the table below before you assume you’re covered.
- ✓It’s the only choice in two counties. In Deschutes and Jefferson counties, PacificSource is the only insurer selling regular Medicare Advantage plans for 2027.
- ✓Out-of-network care has real limits. These plans pay up to $3,000 a year toward elective out-of-network care, and there’s no cap on your out-of-network costs.
- ✓PacificSource isn’t leaving Medicare. It’s leaving the individual health insurance market, which is a different product.
PacificSource's 2027 Plans at a Glance

Cascade Classic Rx is a new plan for 2027.
Medicare’s September 2026 enrollment data shows about 24,000 Oregonians in a PacificSource plan that isn’t on the 2027 list for their county. If that’s you, here’s what to do:
- 1
Read the Annual Notice of Change or letter PacificSource sent you. It tells you whether you’ve been moved into another plan or need to choose one yourself.
- 2
Compare during Open Enrollment, October 15 to December 7, 2026, so your new coverage starts January 1.
- 3
Know your extra rights. If your plan is ending and you aren’t being moved to another one, Medicare gives you a special window from December 8 through the end of February, and Oregon gives you guaranteed issue rights to buy a Medicare Supplement without health questions. Ask SHIBA at 800-722-4134 to confirm what applies to you.
In the Portland area, see Medicare Advantage plans in Oregon and the Portland guide for your 2027 choices.
What You'll Pay at the Doctor
These are 2027 in-network costs from PacificSource’s Summaries of Benefits. None of the four plans requires a referral.
| Service | Oregon Basic | Valley Select Rx | Cascade Classic Rx | Cascade Premier Rx |
|---|---|---|---|---|
| Primary care visit | $0 | $0 | $5 | $0 |
| Specialist visit | $20 | $45 | $60 | $50 |
| Hospital stay, days 1 to 5 | $450 a day | $495 a day | $485 a day | $475 a day |
| Emergency room | $130 | $130 | $115 | $130 |
| Urgent care | $50 | $50 | $40 | $50 |
On Cascade Classic Rx, the $385 medical deductible comes first for some services.
Dental, Vision, Hearing, and Other Extras
| Extra | Oregon Basic | Valley Select Rx | Cascade Classic Rx | Cascade Premier Rx |
|---|---|---|---|---|
| Dental yearly maximum | $2,500 | $1,000 | $500 | $1,000 |
| Major dental work | Covered at 50% | Covered at 50% | Not covered | Covered at 50% |
| Eyewear | $200 every year | $135 every 2 years | $145 every 2 years | $150 every 2 years |
| Over-the-counter allowance | $50 a quarter | $20 a quarter | None | $15 a quarter |
| Acupuncture, chiropractic, naturopathic visits | $10 a visit, 24 visits a year | $25 a visit, 12 visits a year | Not covered | Not covered |
| Emergency care outside the U.S. | Covered, $20,000 limit | Covered, $25,000 limit | Not covered | Covered, $25,000 limit |
All four plans include a $0 routine eye exam once a year, a $0 routine hearing exam, hearing aids through TruHearing at $599, $799, or $999 each, and a $0 fitness membership through One Pass.
Drug Coverage
Three of the four plans include Part D drug coverage.
| Drug cost | Valley Select Rx | Cascade Classic Rx | Cascade Premier Rx |
|---|---|---|---|
| Deductible (tiers 3, 4, and 5) | $600 | $700 | $700 |
| Tier 1, 30-day supply | $0 | $0 | $0 |
| Tier 2, 30-day supply | $6 | $6 | $6 |
| Tier 3 | 20% | 20% | 20% |
| Tier 4 | 29% | 26% | 26% |
| Tier 5 | 26% | 25% | 25% |
On all three, you pay nothing more for covered drugs once you’ve spent $2,400 out of pocket in the year, and covered insulin is capped at $35 a month.
Oregon Basic has no drug coverage, and you can’t add a separate Part D plan to it. It only makes sense if you already have drug coverage that’s as good as Medicare’s, such as through the VA. Otherwise you could owe a late penalty later. See Part D in Oregon.
Going Out of Network
The “POS” in HMO-POS means you can get some care outside the network. The limits are tighter than they sound.
- ✓You usually pay 50%. Most out-of-network services cost you 50% of the bill. Oregon Basic charges a $45 copay for out-of-network primary care, specialist, and physical therapy visits.
- ✓The plan stops paying at $3,000. Each plan pays up to $3,000 a year toward elective, non-emergency care from out-of-network providers.
- ✓There’s no cap on your side. The out-of-pocket maximums in the table above count in-network care only. Out of network, there’s no limit on what you could owe.
- ✓Emergencies are different. Emergency care, urgent care, and ambulance rides cost the same as in network.
Hospitals and Clinics in the Network
PacificSource’s 2027 sales brochures show these health systems and clinics, with a note that providers are subject to change.
| Region | Shown in PacificSource’s 2027 brochure |
|---|---|
| Central Oregon and the Gorge | St. Charles Health System, Adventist Health Columbia Gorge, Providence Hood River Memorial Hospital, Summit Health, Redmond Medical Clinic, High Lakes Health Care, Mosaic, OHSU Health, Legacy Health |
| Lane County | PeaceHealth, McKenzie-Willamette Medical Center, Oregon Medical Group, Slocum Center for Orthopedics, OHSU Health, Legacy Health |
Is St. Charles in network for 2027?
St. Charles appears in PacificSource’s 2027 brochure. Here’s the history that makes Central Oregon readers ask.
- ✓September 2025: PacificSource warned members that St. Charles might leave its network on January 1, 2026.
- ✓November 2025: The two reached an agreement. PacificSource said the Medicare portion covered 2026, and the commercial portion covered 2026 and 2027.
Neither has announced a Medicare agreement for 2027 beyond what the brochure shows. If St. Charles is your hospital, call PacificSource Medicare at 888-863-3637 and ask them to confirm it for 2027 before you enroll.
Is PacificSource a Good Medicare Advantage Plan?
Medicare gives PacificSource’s Oregon plans 3 out of 5 stars for 2027. That’s down from 3.5 stars in 2026, and below the 3.99 national average.
| Rating | 2026 | 2027 |
|---|---|---|
| Overall | 3.5 | 3 |
| Health plan | 3.5 | 3 |

Is PacificSource Leaving Oregon?
No. PacificSource is leaving one line of business, and it isn’t Medicare.
| Line of business | 2027 status |
|---|---|
| Individual and family plans (the kind bought through the Marketplace) | Ending. PacificSource is leaving the individual market at the end of 2026. |
| Medicare Advantage in Oregon | Continuing, with four plans and no Portland-area plans |
| Montana | PacificSource says it’s leaving Montana across its individual, family, and employer plans |
PacificSource announced the changes in 2026, citing growing pressures across the healthcare industry. Oregon’s Division of Financial Regulation confirmed in June 2026 that PacificSource and Providence would both leave Oregon’s individual market at the end of the year.
