Seven of the 32 have no drug coverage. Those only make sense if you already have drug coverage that’s as good as Medicare’s, such as through the VA.

Which Plans Work With Your Health System?

This is the Portland-specific catch. Here’s what each health system and insurer has published for 2027.

If your doctors are atWhat’s published for 2027
ProvidenceUnitedHealthcare says Providence is out of network for its Medicare Advantage plans starting January 1, 2027. Providence’s Portland and Yamhill clinic list shows Devoted Health HMO plans (not the PPO), plus Regence PPO plans at its Canby, Molalla, and Wilsonville clinics only.
OHSU or Hillsboro Medical CenterOHSU says it won’t be in network for Regence Medicare Advantage plans starting January 1, 2027. ATRIO says OHSU is out of network for its Portland-area members. OHSU doesn’t publish a list of Medicare Advantage plans it accepts, so ask before you enroll.
Legacy HealthUnitedHealthcare says a new agreement keeps Legacy in its Medicare Advantage network. ATRIO says Legacy primary care providers are not in its network, except at Legacy Silverton.
Kaiser PermanenteKaiser’s Senior Advantage plans use Kaiser’s own doctors and facilities.
The Portland ClinicSays it will accept Devoted Health and Regence Medicare Advantage plans in 2027, and CareOregon Advantage Plus for existing patients. It says its primary care providers no longer take new patients with Original Medicare.
The Oregon ClinicIts 2027 list includes Aetna, ATRIO, Devoted Health, Regence, and Wellcare. It says it no longer accepts UnitedHealthcare Medicare Advantage plans, and that it’s still negotiating with insurers through the end of the year.

Three honest limits on that table:

  • ✓It’s what’s published, not a guarantee. Contracts can change before January 1.
  • ✓A system accepting an insurer doesn’t mean every plan. Notice how Providence takes Devoted’s HMO plans but not its PPO.
  • ✓Call the doctor’s office with the exact plan name. It takes five minutes and it’s the only answer that counts.

If Your Plan Is Ending

Providence, PacificSource, and HealthSpring members in the Portland area all need a new plan for 2027.

  • ✓Choose by December 7 and your new coverage starts January 1.
  • ✓You get extra time. People whose plan is leaving have a special window from December 8, 2026 through February 28, 2027.
  • ✓You may be able to buy a Medicare Supplement without health questions. That right comes with losing your plan, and it has a deadline.

The full walkthrough is here: what Providence members should do. The deadlines and Supplement rights work the same way for other ending plans. Weighing the two paths? Read Medicare Advantage vs Supplement.

Health Insurance in Portland for 2027

If you buy your own health insurance, 2027 brings four changes at once.

  1. 1

Fewer insurers. Providence and PacificSource are leaving Oregon’s individual market. Portland-area counties still have all four that remain: BridgeSpan, Kaiser Permanente, Moda, and Regence.

  2. 2

Higher prices. The state approved an average increase of 21.6%.

  3. 3

A new website. Oregon is moving from HealthCare.gov to its own Marketplace, Explore Health, at ExploreHealthOR.gov. Enrollment starts November 1, 2026.

  4. 4

A different deadline. Enroll by December 31, 2026 for coverage that starts January 1. Open enrollment ends January 15, 2027.

Here’s what the state’s final rates look like for a 40-year-old in Portland on a Silver plan, at full price before any help paying:

InsurerMonthly premiumAverage approved increase
Kaiser Permanente$59915.8%
Moda$70627%
Regence BlueCross BlueShield$70820.3%
BridgeSpan$73419.3%

Networks matter here too. A few things Portland shoppers should know:

  • ✓OHSU says it won’t be in network for Regence individual plans starting January 1, 2027.
  • ✓Providence’s clinic list shows two individual-market options beginning January 1, 2027: Moda’s Pathways plans and Regence’s PROV marketplace plans.
  • ✓Kaiser plans use Kaiser’s own doctors and facilities.

Had a Providence or PacificSource plan through the Marketplace? The state matched you with a plan from another insurer, and you may be re-enrolled in it automatically. Check its network and price before you accept it.

The silver lining? Many people qualify for help paying, and comparing carefully matters more than ever this year. Learn more about health insurance in Oregon, the Oregon Marketplace, and coverage for the self-employed.

Do You Need a Health Insurance Broker in Portland?

You don’t have to use one. You can enroll on your own through Oregon’s Marketplace or Medicare.gov.

If you want help, Oregon’s Marketplace offers two kinds, and both are free to you:

  • ✓Assisters work for community organizations. They help you apply and enroll, but they can’t recommend a specific plan.
  • ✓Insurance agents are licensed. Oregon’s Marketplace says only insurance agents can recommend which plan might suit you best. It also notes that agents may be paid by insurance companies, so it’s fair to ask.
