How Much Is Health Insurance If You're Self-Employed?

The same as anyone else buying their own plan. Being self-employed doesn’t change the price. Your age, where you live, the plan level, and your subsidy do. For example, for 2027 a 40-year-old in Portland pays $599 to $734 a month for a Silver plan at full price, depending on the insurer. A subsidy can bring that down a lot.

THE TRICKY PART

The Tricky Part: Estimating Your Income

When you apply for a Marketplace subsidy, you estimate your household income for the coming year. For a freelancer, that’s a guess. Here’s why it matters:

1

Your estimate sets your subsidy.

Your subsidy is paid to your insurer each month based on your estimate. At tax time, the IRS compares it to what you actually earned.

2

Earn more, pay it back.

Earn more than you estimated, and you pay back the difference. Starting with 2026 taxes, there’s no cap on how much you may have to repay.

3

Cross 400%, lose it all.

You lose the subsidy completely. For 2027 coverage, that’s $63,840 for one person, $86,560 for two, and $132,000 for a family of four.

TAXES

The Self-Employed Health Insurance Deduction

If you’re self-employed, you may be able to deduct the health insurance premiums you pay for yourself and your family on your federal tax return. The IRS form for this is Form 7206. There are rules about who qualifies and how the deduction interacts with any subsidy, so check with your tax professional.

QUESTIONS

FAQs

What is the best way for a self-employed person to get health insurance?

For most self-employed Oregonians, it's a Marketplace plan, ideally with a subsidy. If your income is lower, OHP or OHP Bridge may cover you for free. A spouse's job-based plan is worth checking first.

How much is medical insurance if you are self-employed?

The same as for anyone buying their own plan. For 2027, a 40-year-old in Portland pays $599 to $734 a month for Silver at full price, before any subsidy.

Can I deduct health insurance if I'm self-employed?

You may be able to, using IRS Form 7206. Check with your tax professional about your situation.

What happens if I earn more than I expected?

You may have to pay back some or all of your subsidy at tax time. Starting with 2026 taxes, there's no cap on repayment.
