Written by Kelly Raulerson · Last updated FacebookEmail
Key Takeaways
- ✓Same plan name, different price. ATRIO Choice Rx has a $5,900 out-of-pocket max in Portland and $7,900 in Salem. Always look up your own county.
- ✓Not every plan is sold everywhere. Prime Rx is only in Douglas, Jackson, Josephine, and Klamath counties. Freedom isn’t sold in Marion or Polk.
- ✓Medicare rates ATRIO’s PPO plans 2.5 stars for 2027. The national average is 3.99.
- ✓Asante is out of network. ATRIO’s provider directory says Asante facilities and providers have been out of network since January 1, 2026.
- ✓OHSU depends on where you live. ATRIO says OHSU is out of network for members in the Portland area and Lane County.
- ✓State supervision is over. Oregon regulators supervised ATRIO’s finances from April 13 to July 14, 2026, then lifted the order.

Going out of network
Because these are PPOs, you can see providers outside ATRIO’s network, usually for a higher cost. Each plan also has a second, higher limit that counts in-network and out-of-network care together.
| Where you live | Choice Rx combined max | Freedom combined max | Prime Rx combined max |
|---|---|---|---|
| Clackamas, Lane, Multnomah, Washington, Yamhill | $5,900 | $4,450 | Not offered |

Every ATRIO plan includes a $0 routine eye exam through VSP and a $0 routine hearing exam through TruHearing. Hearing aids cost $699 to $999 each, up to two a year. The allowances below vary by plan and county, and unused amounts don’t roll over.
| Plan and region | Dental allowance | Over-the-counter allowance | Fitness reimbursement | Eyewear per year |

Drug Coverage
Choice Rx and Prime Rx include Part D drug coverage. Freedom doesn’t.
| Where you live | Choice Rx drug deductible | Prime Rx drug deductible |
|---|---|---|
| Portland area and Lane | $400 | Not offered |
| Marion, Polk | $500 | Not offered |
| Douglas | $500 | $650 |
| Jackson, Josephine | $500 | $350 |
| Klamath | $350 | None |
The deductible applies to drugs in tiers 3, 4, and 5. Tier 1 drugs are $0. On both plans, you pay nothing more for covered drugs once you’ve spent $2,400 out of pocket in the year, and covered insulin is capped at $35 a month.
ATRIO Freedom has no drug coverage. That only makes sense if you already have drug coverage that’s as good as Medicare’s, such as through the VA. Otherwise, going without can mean a late penalty that lasts as long as you have Medicare drug coverage. See Part D in Oregon.
One Freedom perk is county-specific: in Douglas and Klamath counties, Freedom takes $25 a month off your Part B premium.
What Changed for 2027
ATRIO serves the same 11 counties as 2026, with one plan gone: the Prime Rx HMO sold in Marion and Polk counties isn’t back for 2027. For everyone else, the biggest change is how much you could owe in a bad year. ATRIO raised the in-network out-of-pocket maximum on every plan.
| Plan | Portland area and Lane | Marion, Polk | Douglas | Jackson, Josephine | Klamath |
|---|---|---|---|---|---|
| Choice Rx | $5,500 to $5,900 | $6,750 to $7,900 | $5,000 to $7,150 | $6,750 to $7,150 | $6,750 to $7,150 |
| Freedom | $4,150 to $4,450 | Not offered | $4,500 to $7,150 | $6,750 to $7,150 | $5,500 to $7,150 |
| Prime Rx | Not offered | Not offered | $4,200 to $4,450 | $4,150 to $4,450 | $4,200 to $4,450 |

Two more changes worth a look:
- ✓Drug deductibles went up in most regions. Choice Rx rises from $300 to $500 in Douglas, Jackson, and Josephine counties, and from $200 to $500 in Marion and Polk.
- ✓One premium went down. Prime Rx in Jackson and Josephine counties drops from $51 to $43 a month. Other premiums are unchanged.

Is ATRIO a Good Medicare Advantage Plan?
Medicare’s own scorecard gives ATRIO’s PPO plans 2.5 out of 5 stars for 2027. The national average is 3.99.
Medicare rates every Medicare Advantage contract from 1 to 5 stars each year. The score rolls up things like preventive care, managing chronic conditions, member experience, complaints, and customer service.
| ATRIO contract | Counties | 2026 overall | 2027 overall | 2027 health plan | 2027 drug plan |
|---|---|---|---|---|---|
| H7006 | Clackamas, Lane, Marion, Multnomah, Polk, Washington, Yamhill | 2 | 2.5 | 2.5 | 3.5 |

ATRIO and Asante
If you live in Jackson or Josephine County, this is the section to read twice. Asante runs Rogue Regional Medical Center in Medford, Three Rivers Medical Center in Grants Pass, and Ashland Community Hospital.
| When | What happened |
|---|---|
| January 1, 2026 | ATRIO’s Medicare Advantage coverage became out of network with Asante. Both sides say so. |
| February 25, 2026 | Asante said ATRIO owed it about $60 million for care already provided, and pointed to its lawsuit in Jackson County Circuit Court (Case No. 25CV55891). Asante said that after March 31 it would stop offering new routine appointments to ATRIO members, with exceptions. It said emergency, cancer, and behavioral health care would continue. |
| ATRIO’s response | ATRIO has disputed Asante’s claims. It told KOBI-TV that Asante misrepresented the situation and that ATRIO would fight the lawsuit. For 2026, ATRIO said members who see an Asante provider would not pay out-of-network cost sharing. |
| October 8, 2026 | ATRIO’s provider directory still says Asante facilities and providers are out of network. |
The dollar figures are Asante’s claims in a lawsuit that ATRIO disputes. They aren’t a court finding.
What this means for 2027: don’t assume Asante is in network. If Asante matters to you, call ATRIO at 1-877-672-8620 and ask two things. Is my Asante doctor in network for 2027? And does the 2026 promise about out-of-network costs at Asante continue in 2027? Get the answer in writing if you can.
OHSU, Legacy, and Other Networks
ATRIO’s provider directory posts a notice about two more health systems. Here it is in plain words.
| Health system | Where you live | What ATRIO says |
|---|---|---|
| OHSU facilities and specialists | Klamath, Josephine, Jackson, Douglas, Marion, Polk | In network |
| OHSU facilities and specialists | Lane, Yamhill, Washington, Clackamas, Multnomah | Out of network |
| Legacy primary care providers | Any county | Not in network, except those at Legacy Silverton |
| Providence Medical Group clinics, Portland and Yamhill | Portland area | ATRIO isn’t on Providence’s list of accepted Medicare Advantage plans (as of September 1, 2026) |
ATRIO’s notice covers Legacy primary care providers. It doesn’t address Legacy hospitals or specialists, so ask about those directly.

Is ATRIO Financially Stable?
Oregon regulators stepped in this year, and then stepped back out. Here’s the record.
- ✓April 13, 2026: The Oregon Division of Financial Regulation placed ATRIO under an order of supervision. It cited operating losses, inadequate capital and surplus, and a claims backlog that left provider payments unpaid.
- ✓July 14, 2026: The state ended the supervision. It said the conditions behind the order no longer existed and that ATRIO had taken steps to significantly improve its surplus.
Supervision is not the same as a shutdown. ATRIO kept operating throughout, and its 2027 plans are in Medicare’s approved plan file. The state reported about 36,300 Oregonians enrolled in ATRIO plans as of March 31, 2026.
How ATRIO Compares Where You Live
How much choice you have beyond ATRIO depends on your county.
| County | Medicare Advantage plans for 2027 | Insurers |
|---|---|---|
| Multnomah, Washington, Clackamas | 32 each | 8, including ATRIO |
| Yamhill | 27 | 7, including ATRIO |
| Lane | 26 | 7, including ATRIO |
| Marion | 22 | 6, including ATRIO |
| Polk | 19 | 5, including ATRIO |
| Klamath | 6 | ATRIO and PacificSource |
| Jackson | 5 | ATRIO and Aetna |
| Josephine | 5 | ATRIO and Aetna |
| Douglas | 4 | ATRIO and Wellcare |

In Klamath County, PacificSource’s plans cover only four ZIP codes in the north end of the county, so check which insurers serve your address. See every insurer on Oregon Medicare carriers, or compare plans on Medicare Advantage plans in Oregon.
